Purpose

Dr Sarah Mills is an independent healthcare provider, the responsible organisation is South Tyneside and Sunderland NHS Foundation Trust.

The clinic is a specialist service to assess and diagnose developmental, behavioural or learning issues in Children. Diagnoses which may be reached include: Autism spectrum disorder, ADHD, Fetal alcohol spectrum disorder, Learning difficulty or disability, Developmental coordination disorder. In each condition the relevant national or international  guidance (e.g. NICE, DSM-Vor ICD11) will be followed to reach diagnostic thresholds.

Dr Sarah Mills is regulated and licensed to practice by the General Medical council (GMC) registration number 6101767.

Personal data

In order to access the clinic patients must register their details.

We observe the requirements of the Data Protection act 2018, Regulation (EU) 2016/679 and the data protection principles in relation to personal data. By agreeing to these terms, you are agreeing to the processing of personal data to enable us to carry out work on your behalf. You have the right to request details of your personal data held by us.

Fees and Payment

Fees are set taking into account the length of time spent prior to, during and after an appointment to assess reports, collate information, carry out professional liaison and complete reports. As a UK based company all necessary contributions are made to HMRC as well as necessary insurances and running costs. By arranging an appointment you are agreeing to abide by our terms and conditions of business specifically the cancellation policy and are liable to pay costs incurred even if an appointment has not taken place.

A deposit is required to secure the initial appointment. For assessments costing over £300 the deposit is £150, for assessments over £1000 the deposit it £600. This is required at the time of booking. An invoice will be issued for the remaining balance at booking, there is then the opportunity to pay the balance off ahead of the feedback appointment. The final balance is due on the feedback appointment date.

Payments are made by BACS transfer. Card payments are available on request.

Cancellation policy: Any cancellations within 48 hours of the appointment will be charged a 50% cancellation fee that will be invoiced. Please note insurance companies will not cover this charge, and you will be liable. 

If assessments are complete and feedback appointments are missed the full assessment fee is still payable.

Any outstanding invoices will lead to services being ceased until accounts are brought up to date. Dr Sarah Mills reserves the right to leave assessments incomplete and stop further medication reviews and prescriptions until accounts are brought up to date. 

A late payment fee of 10% of the invoice value will be charged on invoices over their terms. Installment plans and flexible payment arrangements can be made on request.

Debt recovery agencies may be required as a last resort if matters cannot be settled through reasonable communication.

If your complaint is not settled through communicating with us we can direct to the relevant organisations that can take your concerns further such as the General Medical council (GMC)
